Sudden Cardiac Arrest

Lindsay’s Law, Ohio Revised Code 3313.5310, 3707.58 and 3707.59 requires that all youth sports organizations provide information to families regarding Sudden Cardiac Arrest.  Please review this information, even if you have already seen it because of another sport in which you participate.

Sign and submit this form to your team manager:  Parent-Athlete-Signature-Form-SCA
      Who should submit the form? A parent or guardian for each player. A copy of the form will be kept in the team binder.

If a player experiences syncope or fainting before, during, or after any practice, scrimmage, or game, the player must be removed from play. The parent and player are responsible for informing the coach of such an event which occurs outside the coach's observation. The player is not permitted to return to play until a written clearance from a physician (MD or DO), a certified nurse practitioner, a clinical nurse specialist, or certified nurse midwife is received by the coach and by the Athens Youth Hockey Association Registrar. A copy of the clearance will be kept in the team binder.

If a player’s biological parent, sibling or child has had a Sudden Cardiac Arrest (SCA), then the player must be removed from play.  The parent and player are responsible for informing the coach and the AYHA Registrar of such a situation. The player is not permitted to return to play until a written clearance from a physician (MD or DO), a certified nurse practitioner, a clinical nurse specialist, or certified nurse midwife is received by the coach and by the Athens Youth Hockey Association Registrar. A copy of the clearance will be kept in the team binder.
